After the upgrade to ver. 1.2.8 r_6440, I couldn't upload photos, permission denied, no thumbnails, etc.
Solution:
Just chmod 777 to the following folders: gallery/1, gallery/1/previews, gallery/1/previews-med
"The record you are trying to create is a duplicate..."
It's better to just uncheck the duplicate check rather than spending hours looking for how there are duplicate records.
Tools > Preferences > Duplicate Checking
